This is a rough draft of the survey I hope to persuade some people to fill out at a SciFi convention this weekend, as a step toward getting more direct data for my dissertation, as my Anthropology professor suggested.
If you have a chance to look this over and offer [constructive] criticism, I'd be very grateful. I MAY have a chance to revise it before I run off copies, or to revise it for later distribution on the internet.
In any case, I've discovered that writing a survey that will secure pertinent data for my dissertation, without inadvertently offending or violating the privacy of the people being interviewed, is a whole lot harder than I'd expected. I should have KNOWN that just because I AM a fan doesn't mean that I know the right way to talk to all other fans. Oh, well.
FANDOM PARTICIPATION QUESTIONNAIRE

On question 5, it's not clear whether you are asking for fandoms that friends and family share with the respondant, or if they just have friends and family whom they know participate in fandom. Also, you may want to tweak it a bit if you mean "friends who predate your participation in fandom"---the dreaded "RL friends" vs. "online friends."
On question 7, if you're meaning to force them to pick THE most important, you should indicate check only one. Either way, you should specify One/As many as applicable, because that could confound your data (i.e., some may pick The One, because they think they're supposed to, even thought they feel that they have two primary ones, etc.)
It's a well-constructed survey, despite my minor nitpicks.

Did you mean to leave out Firefly? I know it's short, so you may well have, but arguably, the fandom was partially responsible for the fact that there'll be a feature film.

What would make it better? Hmm. I'd reduce the number of open-ended questions, since they are a bear to code. However, I think, for purposes of your dissertation, they've gotta be open-ended.....

And you're right: for my purposes I've got to leave lots of open space and hope for any anecdotal evidence people might offer, rather than try to come up with any solid conclusions based on statistical analysis of the results. I'm especially hoping that some people won't be able to fit everything they have to say in the spaces provided and will be amenable to further, live or online conversations on the topic.

Really minor point, but it might be worth swapping the order of question 2 and question 3. Question 3 gives examples of what you mean by participating in the various fandoms, so it might be useful to put this down before you ask people which fandoms they participate in. I know people who repeatedly watch Lord of the Rings and have ploughed their way through even The Silmarillion or whatever but wouldn't necessarily consider themselves to be part of the fandom, but if the questions were the other way round...
Have a feeling it might be useful to mention Star Wars.
Also one aspect you haven't touched is roleplaying, gaming and computer games. Almost every major franchise has spawned computer games, a lot of sci-fi and fantasy standards have related roleplaying games or vice versa in a few cases and then there are other board games etc.
P.S. I might be biased as I met hubby and all the friends from uni that I'm still in contact with because they were either in the Sci-Fi & Fantasy Society or were live-action roleplayers, so RL friends but still over all a sort of broad based fandom clique...

I'm a pal of some of your lj pals. Your survey is very interesting. I'm particulary intrigued by your questions about religion and fandom. If you're open to a suggestion from a stranger, I'd suggest that it would be more neutral to list the choices of religions alphabetically. I have found that someone is always looking to see a religion being privileged or disrespected.
I would definitely answer this survey. I hope you'll put up some of your conclusions when you're ready.
